Abstract

Background: In order to optimize the quality of life of Indonesian Health Card-Health Insurance (JKN-KIS) participants suffering from chronic diseases, Puskesmas
Prambanan in collaboration with BPJS Health organized a chronic disease management program (Prolanis) that focuses on hypertension and type 2 diabetes.
There are still many prolanist clubs in FKTP Puskesmas that are not optimal, so there is a need for monitoring and evaluation. The evaluation aims to find out whether the chronic disease management program that has been implemented, has reached the expected target or not. Health service management is evaluated through inputs,
processes, and outputs..
Methods: This type of research is a qualitative study with an evaluation study design.
The sampling technique in this study is the non probability sampling method, with a purposive specification. Data collection in this study was carried out by in-depth interviews, observations, and document review.
Results: Evaluation results revealed that the availability for prolanis was good, the
number was sufficient. Each officer had competency according to their respective qualifications, although not all prolanist implementing staff at FKTP received prolanist training from BPJS Health, because training was given as a reward to FKTP who achievers. Delivery of reporting on prolanis activities at the Prambanan Community Health Center has been orderly and smooth. There are SK, KK, and SOP that help smooth officers. The availability of infrastructure is sufficient, but the puskesmas hall is narrow, so prolanis activities are carried out outside the building. Prolanist
planning, implementation and supervision are good. Follow-up on prolanis socialization has not been budgeted specifically, so counseling to the general public
has never been done.
Conclusion: Evaluation in prolanis activities at the Prambanan Community Health Center is a number of officers who have never attended training on implementation of
prolanis, there has been no special socialization / counseling to the general public about prolanis in the puskesmas area, where physical activity is less extensive, and participants have not been given a health monitoring book. The number of registered participants and controlled prolanist participants has not met the target. The benefits of joining prolanis are that participants can monitor their health status and increase
knowledge. It is expected that the Prambanan Community Health Center implements the principles of social marketing namely product, place, price, and promotion to increase the number of prolanist participants and the number of visits to prolanist activities at the health center.knowledge / insights on how to live a healthy lifestyle and maintain physical fitness
